I acknowledge that they are not related to the presenter's area of licensure, and are outside of their standard practice area. I acknowledge that we are in a group setting where there are risks involved. I accept these risks and understand that the quality and outcomes of these services may differ from those typically associated with counseling or therapeutic services. I understand and agree that the services provided in this context are not intended to replace or serve as professional counseling or therapeutic services or advice. I acknowledge that no therapeutic relationship is being formed through these services. I understand that by completing this form I am agreeing to these terms and accepting complete liability.

I hearby release and waive against all claims of St. Joseph Parish, its agents, employees, volunteers, representatives, officers, and directors, from any incident relating to my child or children's participation in the babysitting provided at St. Joseph Parish, Maumee.
