I grant permission to St. Joseph Catholic Church to use images or other electronic media of my child taken at youth ministry events for the promotion of the church and the ministries it offers.

Authorization To Allow Electronic Communication Between Adults & Minors:

I hereby grant permission to Bonnie Eicher, Stephanie Stasa, Jacob Peters, Fr. Eric Schild, Middle School and High School Youth Ministry Volunteers, and other church personnel on behalf of St. Joseph Catholic Church Maumee to communicate electronically with my child, with regard to my child's activities and interactions with Life Teen, MSYG, Religious Education, Children's Liturgy of the Word, 6pm Sunday Mass, and other Church activities. Such communications shall be limited to the email address and telephone text number for my child indicated above. All such communications must simultaneously include another adult listed above or myself. The only exception to this policy is in regards to our Parish-Wide Flocknote Messaging System, which is available for any parishioner to subscribe to at any time. This authorization may be revoked at any time by me through written notice to the Parish.

I also give additional permission to the staff listed above to communicate with my child in a one on one text or email conversation pertaining to the ministries and church activities listed above. This authorization may be revoked at any time by me through written notice to the Parish.
